A SECOND INVESTIGATION EMERGES AFTER WILLIAM CHAPMAN’S dєatᩚh: QUESTIONS ABOUT MEDICAL RECORDS

As authorities continue investigating William Chapman’s dєatᩚh at Yorkshire Wildlife Park, a separate development has raised further questions about privacy and professional conduct. Reports published on October 16 said Yorkshire Ambulance Service NHS Trust was investigating two members of staff over concerns that they may have accessed, or attempted to access, information about Chapman without legitimate permission. The development adds a new dimension to a case already involving a coroner’s inquiry, a review of mєntal health care and an investigation into the safety circumstances surrounding the fatal incident.

Chapman died on October 10, 2026, after entering an enclosure containing an Amur tiger named Altai at the Doncaster wildlife park. Emergency responders attended the scene, and the preliminary inquest hearing later heard that Chapman sustained multiple traumatic injuries. The opening of the inquest on October 15 also established that he had a diagnosis of schizophrenia, had experienced previous episodes of psychosis and had been under the care of adult mєntal health services in Doncaster. His family expressed concerns about his contact with those services, prompting RDaSH to commission an independent patient safety incident investigation.

The reported ambulance-service investigation concerns a different issue. Medical information is confidential, and staff access to patient records must have an appropriate professional or operational basis. When concerns arise about possible unauthorised access, an employer may need to establish whether a record was viewed, what information was accessed, why access occurred and whether any rules were breached. In this case, the publicly reported information indicates that two staff members are being investigated over possible access without a legitimate reason. It does not establish that a breach has been proven, that information was disclosed to outsiders or that either individual has been found responsible for misconduct.

The distinction matters because the investigation is at an early stage. An access log may show that a record was opened, but understanding whether that action was justified can require examination of the employee’s role, duties and circumstances. An inquiry must also distinguish between an attempted access and a completed access, and between inappropriate viewing and any subsequent disclosure. Until the trust completes its review and announces substantiated findings, more specific claims about what happened would be speculative.

The issue nevertheless highlights the importance of confidentiality when a dєatᩚh attracts intense public attention. High-profile incidents can lead to widespread curiosity about the person involved, particularly when personal medical information becomes part of court proceedings. But information that is relevant to a coroner’s investigation is not automatically available for unrestricted access by every healthcare employee. Professional confidentiality exists to protect patients and their families, including when the patient has died and public interest is unusually high.

Chapman’s relatives have already spoken about the distress caused by material relating to the incident circulating online. Their appeal for respect comes amid an exceptionally difficult period in which the family is mourning while the circumstances of his dєatᩚh remain under investigation. Any further concern about the handling of private information risks adding another burden, although the outcome of the reported ambulance-service inquiry has yet to be established.

Several separate processes are now relevant to the broader case. South Yorkshire Police and the City of Doncaster Council are examining the incident and safety circumstances; RDaSH is reviewing Chapman’s mєntal health care; and the coroner is preparing for a fuller hearing. The reported investigation into potential access to medical records addresses the protection of confidential information rather than directly determining the cause of dєatᩚh.

The next major date in the coroner’s process is March 18, 2027, when the full inquest is scheduled to take place. In the meantime, the record-access investigation may establish whether any professional standards were breached. The key questions are straightforward, even if the answers may take time: was information accessed, was there a legitimate reason, and were the applicable confidentiality rules followed? Until those questions are resolved, the responsible approach is to recognise the seriousness of the concerns without treating an allegation as a confirmed finding.
