Parents Release School Surveillance Video After 8-Year-Old Daughter diᩚєs from Choking, Demand Greater Accountability

The parents of an 8-year-old girl who died after choking during lunch at her East Texas elementary school have publicly shared surveillance footage of the incident and are calling for answers about how staff responded in the ᑕriᩚtiᩚcal minutes that followed.

Adrian Thompson was eating lunch in the cafeteria at Velma Penny Elementary School in Lindale on April 15, 2026, when she began showing clear signs of distress around 11:40 a.m. Video released by her family shows her standing up from her table, coughing into a napkin, placing her hands near her throat, and walking through the cafeteria while struggling. She approached a supervising adult and gestured toward her mouth in an apparent attempt to signal that she needed help.

According to the Texas Rangers investigative report, the staff member believed Adrian needed to vomit and directed her toward a trash can. The employee then asked whether she needed to use the restroom. Adrian nodded and left the area. She returned moments later still appearing distressed, waving her arms, before running down the hallway toward the nurse’s office.

Inside the nurse’s office, Adrian was described as pointing to her mouth and trying to speak before her face turned blue and she collapsed. The nurse performed the Heimlich maneuver, used an anti-choking device, and began CPR after finding no pulse. Reports later noted that an adult-sized device was used rather than a pediatric one. The school principal called 911 at approximately 11:45 a.m. Emergency personnel arrived within minutes, continued resuscitation efforts, and transported Adrian by ambulance. She was later transferred to Cook Children’s Medical Center in Fort Worth, where she died in the early morning hours of April 16.

The medical examiner ruled the dєatᩚh accidental, caused by choking. The Texas Rangers investigation, closed in July 2026, concluded that no criminal offense had been committed under state law. Adrian’s mother, Anna Thompson, has said the family requested the investigation because they viewed the circumstances as questionable. She has also expressed concern that the inquiry did not include interviews with certain witnesses and left important questions unanswered.

In releasing the video, Anna Thompson stated that if the footage had shown staff immediately recognizing the emergency and beginning to help, the family would still have been devastated by the loss. However, she said the recording does not reflect that kind of prompt response, which is why they chose to make it public. “People need to know about this,” she said.

Lindale Independent School District Superintendent Stan Surratt responded that the district has updated its safety procedures, provided additional staff training, and put new policies in place with the goal of preventing a similar tᩚraɢedy. He acknowledged that no amount of preparation can eliminate every emergency while emphasizing a commitment to continuous improvement.

The case has prompted the family to advocate for broader changes, including possible reforms to laws that grant public school districts certain levels of legal immunity. More than six months after Adrian’s dєatᩚh, her parents continue to seek greater transparency and accountability surrounding the events of that day.
