Two Syringes, a Closed Curtain, and the Protocol That Had No Next Page

Tennessee’s lєthaℓ-inje𝔠ti☉n protocol is built on a single idea: a mᴀssive overdose of pentobarbital will stop breathing, then the heart, inside a predictable window. The state prepares syringes in sets. If the first set does not produce dєatᩚh, a second set is authorized. On September 30 that design was tested in public and failed in a way the paperwork does not describe. Witnesses at Riverbend said the curtain to the media room closed around 7:36 to 7:46 p.m., reopened within minutes, and closed again near 8:05 or 8:06 after a second signal that drugs were being given. Pike could be heard snoring, at times loudly, with her mouth open. One account described her body lifting. The snoring continued behind the closed curtain until the microphone was cut and witnesses were escorted out near 8:53 p.m. Reporters later said officials could not tell them whether the curtain would open again. The Department of Correction’s statement that night did not claim she was ᴅᴇᴀᴅ. It claimed compliance. “The lєthaℓ inje𝔠ti☉n chemical in the protocol has consistently been effective, and the protocol does not allow for additional procedures beyond what was carried out this evening,” the department wrote, adding that Pike had been transported off site.

That silence after the second set is the operational fact that now matters as much as the chemistry. A protocol that ends when the backup syringes are empty leaves executioners, witnesses, and the inmate in a legal and medical gap. Pike’s lawyers said she described burning pain in her arm while still able to speak. Courthouse News, citing media witnesses, reported that she talked, snored, and said her arm was hurting after the injections began. If those observations hold, the procedure crossed from a failed attempt to 𝘬iᩚll into a period of consciousness or partial consciousness under a drug meant to abolish it. Pentobarbital is a barbiturate used in executions by several states and, at times, the federal government. Critics have argued for years that secrecy about compounding, storage, and expiration makes it impossible for courts to know whether a given batch will work as advertised, and that IV failure can turn an intended overdose into a subcutaneous chemical burn. Zivot’s opinion, offered for the defense, is one medical reading, not a state autopsy of the event. The state has not yet published its own account of line placement, flow, or dose delivered into the vein.

This was not Tennessee’s first lєthaℓ-inje𝔠ti☉n problem of 2026. Earlier in the year the state called off an exє𝔠uti☉n because staff could not establish a usable vein. In August, by contrast, Anthony Darrell Hines was declared ᴅᴇᴀᴅ minutes after the first dose began to flow, which is why Pike’s lawyers and outside observers treated the September 30 sequence as a break from recent “successful” cases rather than an isolated mystery. Governor Lee responded on October 1 by postponing the only remaining 2026 date, the December 3 exє𝔠uti☉n of Gary Wayne Sutton, and by ordering what he called a comprehensive third-party review “to determine exactly what occurred.” He said a lawful sentence had to be carried out in a way that was legal, consтιтutional, and effective. On October 3 his office said former U.S. Attorney Ed Stanton would lead the review, a choice that echoes the outside examination Lee used after an earlier exє𝔠uti☉n pause. The same day the governor announced that Correction Commissioner Frank Strada, who had overseen the attempt, would resign later in the month. Strada said the department had followed protocol and that leaving was “in the best interest of Tennesseans.” Pike’s lawyers called the resignation justified and insufficient. “What happened to her Wednesday reflects a systemic failure that goes far beyond any one person,” they said.

The review will have to answer questions the protocol itself ducks. Who placed the lines, and how many attempts were made? Did any line infiltrate? What was the source, concentration, and age of the pentobarbital? When did officials decide the first set had failed, and who authorized the second? At what point did a heartbeat check override the ᴀssumption that snoring meant the process was merely slow? And once the second set was gone and she was still alive, why was there no written medical rescue step inside a procedure the state insists is lawful only if it is also effective? Until those records are public, the fullest timeline remains the one ᴀssembled by journalists and defense filings: a morning stay, an evening reversal, two doses, a woman still breathing, and an ambulance where a hearse had been expected.
